Please note our volunteers are not professionally trained and should not assist with skilled care, medical duties or other things that require a certification, license or special training. If you require specialized or professional care, please refer to another local agency listed on the resources page.

Schedule an In-Home Assessment

It may take a few weeks before services begin, so please call us to make a home visit soon! Contact the office at (765) 423-1879 or fill out the form below to schedule an in-home assessment. We will come to your home and get to know you more so that we may suitably place you with an appropriate volunteer.

*Once the assessment has been done and forms are approved you begin Caregiver Companion Services.

This form is to schedule an in-home assessment. No solicitors, please!
